Most rumors about medical disqualifications are outdated. You CAN drive with diabetes (even insulin), sleep apnea (with CPAP compliance), and high blood pressure (with medication). The key is having the right paperwork: MCSA-5870 form for insulin, CPAP compliance reports, and A1C results.

The DOT Physical: Your Biggest Source of Anxiety

"I want to drive, but I'm scared I won't pass the physical."

You are not alone. The DOT Physical is the single biggest source of anxiety for new drivers—even more than the road test.

Rumors fly around trucking forums: "If you take insulin, you're banned." "If you have sleep apnea, they disqualify you." "If your blood pressure is high, go home."

âś… Here is the truth:

Most of those rumors are outdated. The FMCSA has updated its rules significantly. Conditions that used to be automatic disqualifiers are now manageable—if you have the right paperwork.

1. Can I Drive with Diabetes? (Insulin & Pills)

The Old Rule:

If you used insulin, you were banned or needed a hard-to-get federal waiver.

The New Rule (2026):

Yes, you can drive with insulin. In 2018, the FMCSA removed the complex "diabetes exemption" program. Now, the power is in the hands of your treating doctor.

The "Magic" Form: MCSA-5870

If you take insulin for diabetes, you typically do not need a waiver. You need the Insulin-Treated Diabetes Mellitus (ITDM) Assessment Form (MCSA-5870).

  1. Take this form to your personal doctor (Endocrinologist) before your DOT physical.
  2. Your doctor signs it, confirming your diabetes is stable and you track your blood sugar.
  3. Bring the signed form to the DOT Medical Examiner.
  4. Result: You get a 1-year Medical Card.

What about Pills (Metformin)?

If you control your diabetes with pills and diet (non-insulin), it is even easier. You generally just need to show that your sugar is under control.

⚠️ The Trap:

Most Medical Examiners want to see an A1C test result below 10%. If your A1C is 11% or 12%, you will likely be temporarily disqualified until you get it down.

2. Can I Drive with Sleep Apnea?

"I'm a big guy. Will they force me to wear a mask?"

The Reality: Sleep Apnea is not a disqualifier, but untreated Sleep Apnea is.

If you have Sleep Apnea:

You can drive, but you must use a CPAP machine.

The "Compliance" Rule:

You must bring a report from your machine showing you use it at least 4 hours per night on 70% of nights.

⚠️ The Mistake:

If you show up to your physical without that printed report, you may fail. The doctor cannot take your word for it.

3. Can I Drive with High Blood Pressure?

"I have 'White Coat Syndrome.' My BP spikes when I see doctors."

The Reality: This is the #1 reason drivers fail or get restricted cards.
