Tier 2 (Standard Weight):

The Impact: Carriers with Tier 1 violations saw percentile spikes of 10-20 points overnight. A carrier at 65th percentile could jump to 85th (investigation threshold) based on one expired medical certificate—now weighted at 5 instead of 1.
